    Postmarket cybersecurity

Medical device incident response tabletop exercises

Rehearse a cybersecurity incident on your marketed device before it happens. Part of our postmarket program, scoped to your device and response plan.

Part of FDA postmarket cybersecurity services.

    Example scenarios

Each exercise uses one scenario adapted to your device. These are starting points, not a fixed menu.

    Researcher reports an exploitable flaw

A security researcher contacts you about a remotely exploitable vulnerability in a fielded device and plans to publish in 45 days.

Tests: Disclosure intake, triage, patient-harm assessment, and customer communication.

Known exploited component in your SBOM

A component in your SBOM is added to the CISA Known Exploited Vulnerabilities catalog on a Friday afternoon.

Tests: Exploitability assessment, out-of-cycle patch decision, and compensating controls.

    Hospital ransomware reaches your device

A hospital customer reports ransomware on its network and asks whether your devices are affected or spreading it.

Tests: Evidence preservation, containment guidance, and coordination with the customer.

    Compromised update or cloud service

Your update server or cloud platform shows signs of unauthorized access affecting many patients at once.

Tests: Multi-patient impact, service shutdown decisions, and recovery.

    How it works

    1. Step 1

      Scoping call

Agree objectives, participants, the device and versions in scope, your current response plan, and whether the session is remote or on-site.

    2. Step 2

      Scenario design

We build a device-specific scenario with timed injects from your architecture, threat model, SBOM, and deployment environment.

    3. Step 3

      Facilitated exercise

A discussion-based session, usually half a day, with engineering, product security, regulatory, quality, and customer-facing teams.

    4. Step 4

      After-action report

Response gaps, decision points, action owners, and priorities for updating your incident response plan.

    Sample session outline

A typical half-day session. Timing and injects are adjusted to your scenario and team.

    1. 0:00

      Kickoff

Objectives, ground rules, roles, and the device and versions in scope.

    2. 0:15

      Inject 1: First signal

The initial report or alert arrives. Who receives it, how is it logged, and who is told?

    3. 0:45

      Inject 2: Assessment

Exploitability, affected versions, and patient-safety impact. Who decides severity?

    4. 1:30

      Inject 3: Escalation

The situation worsens. Containment, customer guidance, and communication with regulators and researchers.

    5. 2:30

      Inject 4: Recovery

Patch or mitigation, field rollout, and confirming the fix reached deployed devices.

    6. 3:15

      Hot wash

What worked, what was unclear, and where the response plan did not match what people did.

    7. 3:45

      Close

Agree action owners and next steps for the after-action report.

    Pricing

A tabletop exercise can be included with your postmarket retainer by agreement. Additional or standalone exercises are priced on exercise scope, delivery location, on-site requirements. You get a written price after a short scoping call.

    Frequently asked questions

Is a tabletop exercise included in the postmarket retainer?
It can be. Inclusion is agreed in your retainer proposal. Additional or standalone exercises are scoped and priced separately.
    How is it priced?
Pricing depends on exercise scope, delivery location, on-site requirements. We confirm the price in writing after a short scoping call.
Do you attack our device during the exercise?
No. A tabletop is a facilitated discussion. Live testing of your device is done separately through penetration testing.
Who makes the clinical and reporting decisions?
Your team. We facilitate and record. Patient-safety, recall, and regulatory reporting decisions stay with the manufacturer.
